Dear this error occurs every time i complete the form ( here i have deleted my details ) but when i save the details I receive this

Secret Question   Select Question  Mother's birthplace  Best childhood friend  Favourite teacher  Favourite historical place  Grandfather's occupation   Please select secret question.
